Xylem Volunteer Registration

I hereby grant EarthShare NC permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I hereby authorize the EarthShare North Carolina to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose.

I release and forever discharge and hold harmless EarthShare NC and its successors and assigns from any and all liability, claims, and demands of whatever kind of nature, either in law or in equity, which arise or may hereafter arise from the services I provide to EarthShare NC and participating organizations in the Earth Day Program. I understand and acknowledge that this Release discharges EarthShare NC from any liability or claim that I may have against EarthShare NC with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services I provide to EarthShare NC or occurring while I am providing volunteer services.
